EMT-BASIC COURSE ADMISSION POLICY AND PROCEDURE AND STUDENT HANDBOOK

The course you are applying for will take approximately 4-5 months or less (130-160 clock hours) and will include the AHA Health Care Provider CPR course and CHS Approved HIV-AIDS course. Before you fill out and submit an application, make sure that you will be able to attend all class sessions. This class is limited to a maximum of 28 students. Eligible applicants will be considered on a first come, first serve priority basis. It is the responsibility of the applicant to ensure the following requirements are met prior to application submission. If you have any health-related issues that require reasonable accommodations under the Americans with Disabilities Act to enable you to perform the requirements to function as an EMT student, please contact MAVEN EMS Academy, LLC or the KY Board of EMS regarding this matter.

202 KAR 7:301 Requirements for applicants.

Section 1. EMT Student Eligibility.

(1) Individuals shall be eligible to enroll as a student in an EMT education and training program if the applicant:

(a) Is at least sixteen(16) years of age ; and

(b) 1. Is currently enrolled in grades 9-12 with a minimum GPA of 2.0; or

      2. High school diploma or GED.

(2) The student applicant shall:

(a) Understand, read, speak, and write the English language with a comprehension and performance level equal to at least the ninth grads of education, otherwise known as Level 4, verified by testing as necessary;

(b) Not currently be subject to disciplinary action pursuant to KRS Chapter 311A that would prevent certification;

(c) Meet all additional requirements established by the EMS-TEI; and

(d) Hold a valid motor vehicle operators license or learners permit from a state or territory in the United States.

PURPOSES OF THE COURSE

1. To teach the overall roles and responsibilities of the Emergency Medical Technician in performing both emergency care and operational aspects of his/her job within the EMT scope of practice.

2. To develop skill in diagnosis and all emergency treatment procedures within the EMTs’ scope of practice short of those rendered by paramedical personnel under the direct supervision of a physician.

3. To develop skill in the use of and care for equipment related to the job functions of the EMT.

4. To qualify participants for NREMT certification testing.

STUDENT CONDUCT, CLASS STANDARDS &

COURSE COMPLETION REQUIREMENTS

The following rules and regulations will be carried out without discussion:

1. You are a guest in the building we will be using. All building rules concerning refreshments, breaks, etc. will be carried out. If in doubt ask your instructor.

2. All types of tobacco use is prohibited with the class room and permitted in designated smoking areas only. No smokeless tobacco use is permitted in the class room.

3. Horseplay, sleeping in class, disruptive behavior, “chatter” between students while the Instructor is speaking or teaching or misconduct of any nature will not be tolerated.

4. Class room dress code and hygiene shall be as follows:

A) ABSOLUTELY No clothing that reveals the midriff, breasts, chest, underwear, buttocks or legs higher than mid thigh or is otherwise offensive or revealing.

B) ALL students while in attendance to all class room and class related functions are expected to be in good hygiene and free from all offensive odors.

5. During class session:

a) No student shall be “on call”.

b) No radios are permitted to be “on”.

c) Cell phones are permitted if on vibrate only. If you leave class to talk this may count against class attendance time. At no time will you be permitted to talk on your cell phone while in the class room.

d) No firearms or other weapons are allowed.

e) No visiting EMT’s not on faculty allowed in class without the LEAD INSTRUCTOR’S PRIOR APPROVIAL.

f) No acquisition of CEU’s (except faculty) without the LEAD

INSTRUCTOR’S PRIOR APPROVIAL.

g) No CPR re-certification of non-class members.

h) Students are not permitted to bring children to class sessions.High standards have been deliberately set for this training program. While realizing the effort and sacrifice the students will be making to attend the course, the following standards will be implemented fairly but firmly.

1. The student must achieve a written test score of 80%, practical skills practice participation score of 80% and practical skills/mock boards test score of 80% by t he end of the course for successful course completion and to be eligible to take the National Registry of EMT certification test.

Exams, Tests

A) Five (5) written examinations will be given during this course. ONE missed examination will be allowed to be made up ONLY IF THE ABSENCE IS EXCUSED. Unexcused missed examinations will be given a zero as the test score. Quizzes may also be given in class with or without prior notice. Missed quizzes will not be rescheduled or made up under any circumstance. A score of zero will be given for each quiz missed. Quizzes will be averaged into a single score which will then count as one test toward the students average. Quizzes will be administered solely at the discretion of the Instructor. The student must average 80% or above on the five written exams and quizzes and meet all additional requirements in order to pass the course.

Practical Skills Practice

 B) Practical skills practice is REQUIRED. The student will be subjectively scored by the lead instructor and faculty and must average 80% or above by the end of the course and meet all additional requirements in order to pass the course. If you do not actively and effectively participate during skills practice you will be scored accordingly.

Mock Boards

 C) The student must achieve a minimum score of 80% during the final practical skills/mock boards test at the end of the course and meet all additional requirements in order to pass the course.

Ride Time Internship

 D) Each student shall complete no less than 10 hours ride time. Ride time will be scheduled through the Lead Instructor and only through services approved by this course. All students will comply with the requirements set for by the Ride Time internship agency with which they are scheduled to Ride. Ride time scheduling shall begin after the Patient Assessment sections of the course has been completed. If you are currently working for a service or department you will be permitted to complete no more than ½ of your Ride Time Internship at the service or department for which you work and then only during hours that you are not regularly scheduled for duty. Once you have scheduled ride time with a department you are expected to attend ride time during the time you scheduled. If you are absent from scheduled ride time without prior notice and approval of cancellation you will be counted absent the same as if it were class attendance. There shall be no more than one student scheduled for ride time at any given time at the same service.

There shall be one Ride Time form submitted per day that the student rides. The Ride Time form shall have the date of the ride time, start time and end time, total hours of ride time for the form and the student‘s and proctor’s signatures. All Ride Time must be completed and forms submitted to the Lead Instructor no later than fourteen days prior to the end date of the course.

Dress Code for Ride Time 

Your name badge issues to you that designated you as an EMT Student shall be worn in clear view at all times while engaged in Ride Time Internship activities. Ride time dress code shall conform to agency policy for which you are riding or be dark blue or black uniform pants (NO BLUE JEANS) black shoes and dark shirt. No T-Shirts other than an approved student class T-shirt and ABSOLUTELY NO WHITE SHIRTS OF ANY TYPE ARE ALLOWED.

Patient Contacts

E) Each student shall have made no less than 5 patient contacts during their scheduled ride time. Patient contact forms shall be considered complete when all sections are completed and is signed by the two department employees that were on the run and the student. All patient contact must be completed and forms shall be submitted to the Lead Instructor no later than fourteen days prior to the end of the course.

NOTE: Each patient contact form shall have a minimum of TWO verification signatures from the two department employees on the run and the shift that the student attended ride time hours. IN THE ABSENCE OF THESE TWO VERIFICATION SIGNATURES, THE RIDE TIME FORM WILL BE RETURNED TO THE STUDENT AS INCOMPLETE AND WILL NOT BE CONSIDERED FOR THE TOTAL HOURS UNTIL WHICH TIME THE FORM IS RETURNED TO THE LEAD INSTRUCTOR WITH THE TWO SIGNATURES.

F) A student not meeting all the previous requirements including the tuition and fees payments, attendance, minimum ride time hours, minimum patient contacts, written test, practical skills practice participation and final practical skills/mock boards test requirements cannot graduate from the course and will not be qualified to take the certification test. In this case NO REFUND WILL BE GIVEN.

2. This is a “Learn by doing” course. With good study habits, a sincere effort and participation you should encounter little or no problem successfully completing this course.

3. Effort will be made by the instructors to give you individualized instruction in the event you have a problem with a particular area of the course. It is your responsibility to inform your instructor if you are having a problem.

Attendance

 4. This EMT program is designed so that each student MUST ATTEND EACH AND EVERY CLASS SESSION. Each student will be given a syllabus of the class dates and times. Announcements in class by the Instructor take precedence over the syllabus. It is the student’s responsibility to modify their personal schedule around these dates and times. Attendance will be taken at the beginning of each lesson, therefore each student’s signature is required on each class attendance sheet. The lesson will start promptly at the scheduled time unless delayed by the Instructor. There will be no “make up” classes scheduled unless the entire class was cancelled by the Lead Instructor. No individual “make up” classes will be scheduled for a student that was absent.

Mandatory Classes

The Lead Instructor reserves to right to make any individual class “Mandatory” to attend. In the event a student is absent for a “Mandatory” class it will be considered that they have not completed the requirements of this training program and will not be qualified to take the certification test unless the student can attend the exact class missed at another prior approved course site and can provide proof of their attendance prior to the end of the course or earlier as required by the Lead Instructor.

Tardiness

5. The student that is tardy three times will be given one absence on the attendance record. The student is considered tardy if they are not in the classroom when the lesson begins. If you are late it is your responsibility to sign in on the day of class. If you are late for class and forget to sign in on the attendance sheet before the end of that class you WILL BE CONSIDERED ABSENT AND WILL NOT be allowed to sign in at a later date without prior authorization of the Lead Instructor.

Emergency Absences 

 6. Emergency absences will be considered on an individual bases. Proof of reason for absence may be required at the Lead Instructor discretion.

TWO EMERGENCY absences will be permitted during the course. Written proof for emergency absences may be required at the Instructor’s request. Once you have accrued TWO absences you will be placed on attendance probation. The following will be considered emergencies.

A) Any incident that leads to the student seeking treatment in an emergency room or by a physician for which they have a doctor’s excuse.

B) Any incident that leads to the student’s direct family member seeking treatment in an emergency room. Direct family members are considered as follows: The student’s Spouse, Son, Daughter, Grandchild, Brother, Sister, Parent, Grandparent, Stepchild, Stepparent, Stepsister, Stepbrother or other person considered on an individual basis approved at the sole discretion of the Lead Instructor.

C) Absence due to attendance of a funeral of any direct family member (see above).

D) Hospitalization of the student.

E) Jury duty during class session hours. (Not before or after class hours)

F) Subpoena or summons for court appearance during class session hours.

G) Attendance to a required military function.

H) If you are a volunteer or paid member of an Emergency Medical Service, Police Department, Rescue Squad or Fire Department and are called to respond or assist on an emergency scene PRIOR TO THE BEGINNING OF CLASS and are detained during class time.

I) Any incident that the Lead instructor considers an emergency. This will be considered on an individual basis.

NOTE: The THIRD absence regardless to the reason may disqualify the student from completing the course requirements. The recommendation to allow the student to complete the course will be based upon current academic standing, quality of class participation, ability to perform practical skills that have been covered thus far in class and validity of reason for the absences and will be made at the discretion of the Lead Instructor.

7. A student may be expelled from class and may be reported to the appropriate agency if any of the following occurs:

A) The student is caught cheating on any class administered exam.

B) The student violates any of the rules set forth under the Student Conduct & Class Standards.

C) The student falsifies documentation regarding Field Internship Ride Time or Patient Contact Information.

D) Other reason as deemed appropriate and necessary by the Lead Instructor and/or Course Coordinator.

Successful completion of the course qualifies you to take the National Registry of EMT certification exam. Successful completion of the course does not guarantee that you will pass the NREMT exam. Successful completion of the course signifies that you have satisfactorily completed the course that made you eligible to take the NREMT exam.

It will be your responsibility to maintain your National certification if you pass. This requires that you obtain 72 hours of specific continuing education subjects within every 2 year certification period. Visit www.nremt.org for more information.

TUITION and OTHER FEES

(These are subject to change with or without notice)

Tuition: The tuition for this course is $ 450.00. This includes the HCP CPR course and card. This is for tuition only and does not include the cost of any of your required Text Books and Work Books, Lab kit, State or National Registry applications or State and National Registry testing fees.

Tuition Scholarship Recipients: Potential students must meet all the requirements and be accepted for enrollment for consideration. One tuition scholarship recipient per class will be chosen from the completed scholarship applications received as outlined on the scholarship application and information form. Tuition scholarships will be for tuition cost only and will not cover items such as books, lab kits, testing fees, etc. If the student does not complete the course the student shall be required to repay the tuition scholarship monies with in thirty days from the last date of attendance. In the event of extenuating circumstances that inhibits the students ability to repay the tuition scholarship, or the student is unable to attend as outlined in the emergency absence section of this hand book, MAVEN EMS Academy, LLC shall consider in an individual basis the requirement to repay the tuition scholarship. If MAVEN EMS Academy, LLC decides the student does not meet the requirements for emergency absences or does not have extenuating circumstances the student shall be required to repay the tuition scholarship if they do not complete the course.

Tuition cost is due in full no later than class session number two unless you have approved prior WRITTEN PROOF OF OTHER TUITION COST ARRAINGEMENTS and these arraignments have been verified by MAVEN EMS Academy, LLC. Collection of tuition will be during the first class meeting. Payment of tuition will be accepted for the exact amount in the form of CASH, CERTIFIED CHECK, CASHIERS CHECK, or MONEY ORDER made payable to MAVEN EMS Academy, LLC. No personal checks will be accepted.

NOTE: If tuition in full or tuition arraignments have not been met as outlined above by the beginning of class number three, you will be dropped from the course roster. You will not be permitted to take your certification exam until all your course obligations, including financial obligations have been met.

Refund Policy: 100% tuition refund if the student officially withdraws in writing before the first class. 50% refund minus HCP CPR course fees ($50.00) if the student officially withdraws in writing after the first class but before the third class. 25% refund minus HCP CPR course fee ($50.00) if the student officially withdraws in writing on the third class but before the fourth class. * If the student withdraws on the fourth class and beyond there is no refund. No refund will be given for Text or Work Books if their wrappings have been opened.

* NOTE: If full tuition has been paid and at any time after the fourth class a student is called to active duty or is required to miss class due to military functions, and it causes the student to fail to meet the requirements for successful completion of this course, the student will be given first priority acceptance and the option to attend a future class being conducted within four years of the beginning date of their original course. (verifiable proof of cause to miss may be required) The student’s tuition will be considered “Pre-paid” in full…as a “Thank You” For giving me the freedom to teach you this course.

Books and lab kit: We will be using the AAOS Emergency Care and Transport of the Sick and Injured Latest Edition, Text and Workbook. Do not order these books until you are accepted for enrollment. Once accepted for enrollment, you will be responsible for purchasing the Basic Text and Workbook. It may be possible that they can be ordered by MAVEN EMS Academy, LLC at the discounted price. UNDER NO CIRCUMSTANCE WILL TEXT BOOKS OR WORK BOOKS BE DISTRIBUTED BY MAVEN EMS ACADEMY, LLC PRIOR TO THE BOOKS BEING PAID FOR.

Once accepted for enrollment you will need to purchase the following items for your lab kit: Stethoscope, Blood Pressure Cuff, CPR Microshield or other CPR Barrier, note taking material and writing utensils.

Additional Fees: (All fees subject to change without notice)

As set forth by STATE and NATIONAL GUIDELINES, Monies for the following fees are accepted in the form of CERTIFIED CHECK, CASHIERS CHECK, or MONEY ORDERS ONLY. No personal checks or cash will be accepted. NO EXCEPTIONS!
